The end-to-end platform links acute and post-acute EHRs, providing visibility to providers, payers and ACOs of the care patients receive across care settings so that all providers can coordinate patient care efficiently and effectively.&nbsp;&nbsp;<\/p>\n\n\n\n<p>In 2016, CarePort was acquired by\u00a0<a href=\"https:\/\/www.allscripts.com\/\" target=\"_blank\" rel=\"noreferrer noopener\">Allscripts<\/a>\u00a0and on 13 October 2020, WellSky acquires WellSky.
